Liability Release and Waiver Children’s Event — Hosted by Hilltop House of Prayer at Piedmont Park Alliance Church

Please read this waiver carefully before allowing your child to take part. By signing, you agree to the terms below on behalf of yourself and your child.

Who this covers. “Event” means the children’s event hosted by Hilltop House of Prayer and held at Piedmont Park Alliance Church. “Released Parties” means Hilltop House of Prayer, Piedmont Park Alliance Church, and their pastors, staff, volunteers, and leaders. “Child” means the minor you are registering. “Parent” means you, the parent or legal guardian signing this form.

Permission to take part. I give my child permission to attend and take part in the Event and its activities.

I understand there are risks. The Event includes group activities, games, and play. I understand that taking part has risks, including minor injuries and, though unlikely, more serious harm. I accept these risks on behalf of myself and my child.

Release of claims. To the fullest extent allowed by law, I release the Released Parties from any claims, demands, or lawsuits for injury, illness, loss, or damage that my child or I may suffer in connection with the Event. This applies except where harm is caused by gross negligence or willful misconduct.

Promise not to hold them responsible. I agree not to bring a claim against the Released Parties for any covered injury or loss, and I will cover any costs they face from a claim brought on my child’s behalf, to the extent the law allows.

Medical care. If my child needs medical attention and I cannot be reached, I give the Released Parties permission to seek emergency medical care for my child. I am responsible for any medical costs.

Health and allergies. I confirm my child is well enough to take part. I have shared any allergies, conditions, or special needs the leaders should know about.

Photos and video. I give permission for photos and video taken at the Event to be used by Hilltop House of Prayer and Piedmont Park Alliance Church for ministry, promotional, and social media purposes, without payment to me. (Remove this section if you’d rather collect photo consent separately, or add an opt-out checkbox in the form.)

My agreement. I have read this waiver and understand it. I am signing freely, on behalf of myself and my child.
